F01 · School needs assessment
The starting point for any school we work with. It records the school's profile — learner numbers, grades, how much printing and paper access it currently has — and then what it actually needs, whether that is paper stock, printed worksheets, stationery, books or art materials. We complete it with the school before we promise anything, so that support matches the real need.

F02 · School support approval
The decision that follows an assessment. It records whether support was approved, deferred or declined, what exactly was approved, and who signed it off. It exists so that a school always gets a clear answer, and so our board can see why each decision was made.

F06 · Resource and printed worksheet delivery record
Signed by the school when a delivery arrives, confirming what was received and in what quantity. It is the proof that a donation reached a classroom, and it is what we draw on when reporting back to the business or sponsor who made it possible.
For sponsors and donors

F03 · Corporate sponsor and donor form
For a business or individual offering support. It captures your details, the kind of support you are offering — paper, printing, funding or materials — and any restrictions you want placed on how it is used, before we accept or allocate it. Filling it in first avoids the awkward situation of a donation being used for something you did not intend.

F04 · Sponsor a school agreement
A short, plain-language agreement for sponsoring a particular school. It sets out who the sponsor is, what support has been approved, the principles both sides agree to, and what reporting you can expect back from us. Two pages, not twenty.

F05 · Paper and materials donation and collection record
Completed when we collect paper or other materials from a printing company or donor. It records what was collected, roughly how much, and when — creating a traceable line from your loading bay to a classroom. It is also the record a participating school relies on when issuing a section 18A certificate.

F07 · Children's artwork and publicity consent
Our Art for Education work showcases and auctions artwork made by learners, and no child's artwork, name, image or story appears anywhere without written permission from a parent or guardian first. This form is that permission. It sets out exactly what may be used and where, keeps identification to a minimum, and can be withdrawn at any time — see clause 3.7 of our Privacy Policy.
These forms sit alongside our policy documents. If you are looking for the PAIA manual, Privacy Policy, Terms of Use or Email Legal Notice, they are on the Legal page.
